Why stomach sleeping is hard on the lower back
Lying face down puts the heaviest part of the body, the pelvis and abdomen, at the centre of the mattress with nothing holding it up but the surface. The lower back arches to follow it, the head turns ninety degrees to one side to breathe, and the shoulders are pinned forward.
Stomach sleeping is the position most often recommended against for the spine, and also the one people find hardest to give up, because it is where they fall asleep fastest.
What happens over a full night face down
On a surface that is too soft, the hips drop an inch or more below the chest and the lumbar spine hyperextends all night. The result is a stiff, sore lower back on waking that eases once you are upright, and a neck that aches on whichever side you turned it to.
Stomach sleepers on the wrong bed also pull their arms up under the pillow to prop the chest, which loads the shoulders. The surface matters because how far the pelvis sinks is set almost entirely by the mattress.
What to look for in a mattress for stomach sleepers
Support first: how much hip sink is too much
Face down, the question is how far the hips drop relative to the ribcage. Half an inch is fine. An inch and a half means the lower back is arched all night. The support core sets this more than the comfort layer does.
In a hybrid, look for 13.5 to 14.5 gauge coils, where the lower number is the thicker, stiffer wire. In an all-foam bed the base layer should be at least 1.8 lb/ft³ polyfoam. A soft base cannot be rescued by a firm top.
Comfort layer depth: keep it thin
Thick comfort layers are what stomach sleepers should avoid. Three inches of memory foam feels lovely for the first minute and then lets the pelvis settle into it. Two inches or less of a springy foam, latex, or a firm polyfoam over the support core is the right range.
That layer should take the edge off the knees, ribs and hip bones without creating a hollow for the abdomen. If a mattress lists a comfort layer of four inches or more, it was built for a side sleeper.
Zoning: firmer under the middle third
A zoned mattress puts stiffer coils or denser foam under the middle third of the bed, where the hips land. For a stomach sleeper this is the single most useful construction feature, because it lets the maker keep the shoulder and leg zones a touch softer while still holding the pelvis up.
Look for it described as lumbar zoning, centre-third reinforcement, or a three-zone or five-zone coil unit. Check where the zone actually sits. On a short person the hips can land above it.
The pillow fix, and why to try it first
Before spending on a mattress, try two cheap changes. Swap your head pillow for a thin one, under three inches, so the neck is not bent upward as well as sideways. Then put a flat pillow under the pelvis. That one pillow lifts the hips and takes the arch out of the lower back on a surface that is only slightly too soft.
If it fixes the morning stiffness, the bed you have is closer to right than you thought. A sagged mattress cannot be fixed from the top.
Durability: a firm bed that softens is the worst outcome
Stomach sleepers depend on the mattress holding its shape, because the whole point of buying firm is lost once the centre goes soft. Foam softens under body heat and repeated compression, and lower-density foam softens faster. Look for 3.0 lb/ft³ or more in any memory foam layer and 1.8 lb/ft³ or more in polyfoam.
Heavier sleepers should look harder at latex or coil-heavy hybrids. Rotate the mattress head to foot every three months for the first year to spread the wear.
If you do not stay on your stomach all night
Back sleeping
Most stomach sleepers roll onto their back at times. A surface firm enough for face-down sleeping will be at the firm end for back sleeping, but workable, since back sleeping also wants the hips held up.
Side sleeping
Side sleeping is where a stomach sleeper's mattress bites. The shoulder has nowhere to go on a firm surface. If you spend time on your side, drop a few points of firmness and accept a little more hip sink.
Combination sleeping

Mistakes stomach sleepers make when buying
Trusting the word firm on the label
Firm means different things to different makers, and a firm memory foam bed can still let the hips sink. Judge the support core, the coil gauge and the foam density, not the adjective on the box. The number on our scale is a better guide than the marketing name.
Buying a plank to fix a sagging bed
People replace a sagging mattress with the hardest thing they can find and wake with sore ribs and knees. The fix for sag is a proper support core, not a slab. A surface around 65 to 72 holds the hips without turning the front of the body into a pressure point.
Keeping the thick pillow
A stomach sleeper on a five-inch pillow has the neck bent back as well as turned to the side. No mattress corrects that. Move to a thin pillow or none at all, and the neck complaint often disappears before the new mattress has even arrived.
Deciding in the first few nights
A firmer bed feels wrong for a week or two if you have been sinking into a soft one for years. Give it the full 30 nights before making up your mind. Morning back stiffness that eases as the trial goes on is the surface doing its job.

Is memory foam bad for stomach sleepers?
Deep memory foam is. A thick, slow layer lets the pelvis settle into a hollow and holds it there. A thin memory foam layer over a firm core is fine, and so is a dense memory foam of 4 lb/ft³ or more in a two-inch layer. Latex and hybrids are usually the safer choice.
Can a topper make my soft mattress work for stomach sleeping?
Rarely. A firm topper on a soft mattress still sinks with the mattress under it, so the hips drop just as far, only from a higher starting point. Toppers add softness well and firmness badly. If the mattress has a dip in the middle, the answer is a replacement, not a layer on top.
Should stomach sleepers use a pillow at all?
Under the head, a very thin one or none. Every inch of pillow bends the neck further back while it is already turned sideways. Under the pelvis, yes: a flat pillow there lifts the hips and takes the arch out of the lower back, and it is the cheapest change a stomach sleeper can make.
